Tip 3 – Get connected
- Make contact with young people early and make sure they know who is out there and who they can contact for support and practical assistance.
- Make sure young people are connected with relevant people at school:
- Careers adviser
- Counsellor
- Year adviser
- If necessary, link young people with other specialist teachers, for example:
- Transition teachers - may be called Support Teachers (Transition) or Transition Officers
- Support Teachers
- Head Teachers Welfare
- Help young people and their families make links with disability staff at post-school settings they are considering, for example disability services at TAFE and university, and Disability Employment Network providers. It is important that these connections are made early.
- Identify and support linkages between community organisations and service providers working in transition. Work towards seamless links, coordinated processes, shared information, and minimal duplication. It is important that organisations are aware of each others' roles and purposes. Make contact with your local NDCO if you need further help or information. They can help to support linkages between schools and services.
- Encourage young people to keep a record of the names and contact details of people who have been helpful to them – as they may be useful contacts in the future.
- Encourage young people to talk with their family, friends, and other people with disabilities about their experiences after school and about help that is available.
